Only for cardiovascular risk reduction indication (FDA-approved March 2024). Weight-loss indication remains excluded from Medicare Part D by statute.
Medicare Part D coverage of Wegovy in 2026 follows specific rules: (1) Cardiovascular risk reduction — COVERED. Requires established cardiovascular disease (prior MI, stroke, or revascularization) plus overweight/obesity. Use the ICD-10 cardiovascular code, not the obesity code, on the prescription; (2) Weight-loss-only indication — EXCLUDED by Medicare Modernization Act statute. Cannot be appealed to coverage; (3) Type 2 diabetes — covered when prescribed as Ozempic (same molecule, T2D label). The July 2026 CMS expansion for "obesity with serious comorbidity" applies on some Medicare Advantage plans but not Original Medicare. Check your plan formulary. Without coverage, NovoCare direct cash-pay ($499/mo) is the most accessible path. Some Medicare beneficiaries use the cardiovascular pathway specifically to unlock coverage.
